I understand that the High Voltage Athletic Association is an all-volunteer organization and as the parent or guardian of the above named participant do hereby agree to assist in the operations when requested as outlined below:
1. I understand that parents may be required to work a turn in the concession stand when requested - normally one shift per season.
2. I understand that parents may be asked to help with field duty and cleanup of the park itself.
3. I agree to be responsible for all HVAA uniforms and/or equipment issued to the above named participant or myself.  I agree to return any equipment upon request and reimburse HVAA for any lost or damaged equipment issued to the above named participant or myself.
